Job Overview: Reporting to the Clinic Manager, the RN works to full scope of practice through the application of knowledge to “assist clients in meeting their health needs in whatever setting, complexity and situation they occur throughout the life span. Scope of practice includes all the interventions that registered nurses are authorized, educated and competent to perform. It is limited by the specific competencies of the individual registered nurse to perform the activities necessary for the client population with whom that nurse practice, within applicable legislation and requirements of the employers as described in employer policies.” — College and Association of Registered Nurses of Alberta, Scope of practice for Registered Nurses, 2011. This is a permanent, full-time position.
Key Responsibilities

Chronic Disease Management: Assess and support chronic disease management, guiding and empowering clients in managing their health.

Tobacco and vape cessation education and non-judgmental support providing referrals, education, and linkages to programs for patients interested in cessation.

Mental health and addiction screening and providing support, education, resources, and referral when appropriate.
-
Reproductive Health: Provide inclusive, age-appropriate care related to contraception, menstrual health, STIs, and healthy relationships. Offer confidential support and make referrals when needed.

Gender Care: Provide non-judgmental, inclusive care for youth exploring gender identity. Support them in accessing gender-affirming care and relevant resources.

Complete appropriate health questionnaires e.g. MOCA, ASQ, PHQ-9, GAD-7, Vanderbilt to assess mental and physical health needs.

Best possible medication history: Works with clients to create a complete, accurate, and up-to-date medication list.

Advanced Care Planning: Provide guidance and clarity to clients and families as they plan for future health needs.

Quality Improvement: Develop an understanding of the principles of quality improvement, apply quality improvement principles in the provision of care, participate and initiate the implementation of improvement processes within primary care that support improving patient care.

Collaborate effectively with the multidisciplinary team to achieve change and improvements in patient care.

Support and mentor team members and colleagues in the principles of quality improvement.

Use data to inform decisions about improving patient care both at the patient population and individual care level.

System Navigation: Support clients in finding appropriate resources.

Perform other duties as assigned.

Qualifications and Experience:
-
Graduate of an accredited degree program in nursing.
-
Active practice permit with the College of Registered Nurses of Alberta (CRNA).
-
3-5 years recent experience in a primary care setting.
-
Experience working with youth ages 12- 24 is an asset.
-
Experience and knowledge in providing addictions and mental health support.
-
Class 5 driver’s license, a personal vehicle for business use and a clean 3-year drivers abstract are required.
-
Basic Life Support (BLS) within 30 days of hire date; certification must be valid for a minimum of 6 months upon hire.
-
A satisfactory vulnerable sector criminal record clearance and child welfare intervention check (for CHC and YHC only) are required.

Demonstrated knowledge, understanding and commitment to the Truth and Reconciliation Commission of Canada: Calls to Action is an asset.
Work Environment and Hours:

Predominantly outreach role; most work will be done at school sites and community sites.

Standard Hours: 8:30 am – 4:30 pm, Monday to Friday.

1.0 FTE (37.5 hrs/week)

Requires extended periods of sitting, with some standing and walking (sometimes up and down stairs).
-
Involves physical action requiring the ability to bend/crouch and reach above shoulder height.

Outreach or home visits as needed.
-
Occasional driving may be required as part of work duties.
-
Occasional time outside in all weather conditions.
Driving on behalf of The Alex:
This position requires the use of a personal vehicle for work-related purposes. Candidates must show proof of a class 5 driver's license, a personal vehicle, and a clean 3-year drivers abstract. Candidates must also agree to maintain $2 million liability insurance on their personal vehicle, and consent to the possibility of an annual driver's abstract review to meet our insurance provider’s requirements. Mileage will be reimbursed in alignment with the CRA guidelines.
