Registered Practical Nurse (RPN) – Unionized Position 0.8 FTE (30 hrs./week)

· Assists with direct patient care procedures and related tasks; check in patients, assist with obtaining relevant patient history, document presenting health issue(s), obtain vital signs, prepare the chart and assist with medical examinations
· Keeps exam rooms adequately stocked with required supplies and prepares rooms for procedures.
· Performs duties outlined by Medical Directives independently.
· Provides telephone and in-person triage according to established practice protocol. If uncertain, checks with the Provider.
· Assists and supports patients with implementation of the health plan including interventions and diagnostic testing as directed by the patient’s primary care provider.
· Determines the need for and initiates consultations with other services and health-care providers in a timely and supportive manner as directed by the patient’s primary care provider.
· Co-ordinates services and care with the patient to ensure continuity and follow-up.
· Monitors and maintains infection control practices according to established standards.
· Assesses the immunization status of all patients and ensures immunizations are current.
· Works collaboratively with the healthcare team regarding preventative care monitoring and screening
· Builds on patient and community resources in planning health promotion and illness prevention strategies.
· Provides supervision, mentorship, orientation, observational and teaching opportunities to students ensuring that program goals and objectives are met.
· Demonstrates leadership in areas such as chronic disease management, reproductive health, shared care models, safety and health.
· Establishes and maintains effective professional relationships and partnerships with other organizations to benefit system integration, efficient service utilization, effective collaboration and optimal patient care
· Completes documentation, accurate statistical data, and reports.
· Participates in the maintenance of an accurate comprehensive health record.
· Documents according to standards for completing electronic medical records.
· Completes patient forms/documents as directed by the Primary Care Provider, when appropriate.
· Maintain medication room (maintain log, check for outdates, arrange for drug rep lunch and learn presentations)
· Immunization fridge management – temperature log, check for outdates, inform management if immunizations need to be ordered that are not provided by Public Health.
· Supplies management – stocking of exam rooms, checking for outdates, informing management if supplies need to be ordered.
· Utilizes all health-care encounters as an opportunity to identify educational needs and provide health education (e.g., immunizations, screening, health lifestyle, smoking)
· Diploma Registered Practical Nurse
· Current CPR and First Aid
· Current registration with the College of Nurses of Ontario.
· Current membership with RPNAO (Optional)
· Eligible to work in Canada/Ontario
· Certification in the administration of immunizations (or demonstration of competency)
Desirable Qualifications
· Phlebotomy experience considered an asset
· Experience working in a family practice, community or primary health care setting
· Experience in working within a collaborative interdisciplinary team

Compensation: $25.50 - $28.76
Rates subject to change due to current negotiations
