We're looking for someone to join our team as a
Supervisor CPHW who:
The CPHW Supervisor is responsible for the direct supervision of staff and the implementation of the specific components of the program. The Comprehensive Perinatal Services Program provides comprehensive prenatal care to our obstetric population. The CPHW Supervisor monitors the case management of patients, especially the complex, high risk cases, and provides staff with specific program requirements. The program supervisor works as a member of the interdisciplinary team which delivers the components of the program.
Essential Functions:
- Responsible for supervision of staff and for staff meeting program requirements.
- Responsible for monitoring the case management of patients; provides direction and consultation to the staff in the case management of patients.
- Interview, select, and train new staff, conduct employee reviews, and coordinate training.
- Assists in the orientation and training of new employees.
- Organize team roles and evaluate employee performance on a yearly basis.
- Approve payroll, manage time off requests, and optimize resource productivity.
- Manage staff by assigning and delegating tasks as needed.
- Participate in case conferences, staff meetings and program planning.
- Assists in the coordination of the patient’s care with other health professionals as wells as community agencies.
- Ensure accurate and timely maintenance of patient information, records and data bases.
- Responsible for reporting requirements of programs/projects.
- Attends local, regional, and state meetings at the Program Director’s discretion.
- Utilizes interview format to conduct prenatal and medical histories, and educational, psychosocial, and nutritional assessments following established protocols.
- Provides support and health information as indicated by the assessments as well as referrals to community agencies and resources.
- Identifies high risk patients and refers according to established protocols.
- Documents in the patient’s medical chart and develops the care plan after the initial prenatal history.
- Monitors patient’s attendance to prenatal appointments, contacts patients to reschedule any missed appointments and documents in the patient’s medical charts any contact made with patient.
- Is able to be resource to the patient for problems and questions. Assists the patient in problem solving.
- Conducts patient trimester and postpartum assessments, completes assessment forms and up-dates care plan following CPSP guidelines.
- Provide patients information and education on all methods of birth control and chart services provider in the patients’ medical record.
- Interacts with medical providers, nursing staff, nutritionist, and other team members for interdisciplinary team approach to patient’s care.
- Perform introductory and annual employee reviews, competency forms, and provide constructive feedback on their performance to help meet organizational and professional goals.
- Reasonable and predictable in person attendance.
- Generated charges for services rendered.
You'll be successful with the following qualifications:
- Minimum one-year relevant work experience in related field.
- Knowledge of community resources desirable.
- Knowledge of socio-economic environmental and cultural factors affecting patient’s health care.
- Ability to work effectively in an interdisciplinary team approach with both professionals and paraprofessionals.
- Ability to communicate effectively both orally and in writing with persons from diverse social, economic and cultural backgrounds.
- Ability to maintain patient dignity and confidentiality.
- Bilingual and biliterate in Spanish desirable.
- Willingness to work non-regular hours as required.

- Must adhere to Clinica Sierra Vista’s employee health/immunization requirements or provide a valid exemption request for subsequent approval.
