Position Summary
Reporting to the client Nursing Manager, the Field Case Manager (FCM) is responsible for managing all aspects of program support for assigned physicians in a given geographical area. The FCM will enroll patients into the patient support program (PSP), provide vital patient support, advocate for the patient needs, and coordinate the care and treatment of patients with complex medical conditions who require specialty medication. With a general understanding in regional government payer policies and procedures, the FCM supports the regional needs of the client and works directly with the in-house team to facilitate reimbursement, scheduling and pharmacy dispenses to ensure patients have easy access to their prescribed therapy. The individual also assists the physicians in assessing the needs of each patient resulting in increased patient program enrollment and improved patient outcomes.
Primary Duties And Responsibilities
Practice Management & Clinical Skills
- Adheres to the College of Nurses standards and nursing practices
- Tailors program components to address the specific needs and communication preferences of assigned prescribing physician
- Evaluates patient status and collaborates with the prescribing physician to complete patient enrollment
- Partners with the reimbursement specialist to complete special authorization and public payer forms to secure patient coverage, by analyzing patient charts, assessing prior therapies, diagnostic tests, and disease state scores
- Actively evaluates the patient file, with the ability to predict next steps, and offers recommendations to improve outcomes and enhance service quality
- Proficient in conducting patient assessments, including medical history reviews, disease activity, and symptom management
- Maintains accurate and thorough documentation of patient assessments, interventions, and outcomes
- Documents in client management system.
Patient Support
- Completes an enrollment call for each patient upon receiving the request from the prescribing physician and gathers necessary patient information
- Provides patient education on the specialty medication, therapy guidelines and next steps
- Keeps physician updated on the status of their patient’s file
- Demonstrates patient-centered care by respecting patient preferences, values and cultural backgrounds
- Engages patients in shared decision-making and treatment planning
- Coordinates patient services in line with KPIs ensuring quick access to therapy
- Works closely with the reimbursement team to manage private and public coverage for the patient
- Follows up with patients, pharmacists, and insurers on the status of the reimbursement process
- Guides patients through financial assistance options and provides necessary information
- Assists patients with pharmacy selection and drug delivery coordination
- Supports the patient on therapy and monitors the patient ongoing
- Reports Adverse Events in compliance with program operating procedures
- Identifies trends in barriers to coverage and communicates them to their Manager directly.
Mentorship/Peer Support
- Assists in the training and development of the new Field Case Managers as needed
- Offers support and assistance to peers through consistent and ongoing communication
- Cultivates and maintains a positive relationship with the PSP team
- Provides timely patient information as required using the program designed communication channels.
Client Management/ Customer Interaction
- Attends and participates in in-person meetings with assigned geographical physicians with Sales Representatives
- Proactively visits physician sites to engage customers and maintain positive relationships
- Develops strong working relationships with physicians and their office staff, providing frequent updates on patient files, the enrolment process and on drug start date
- Provides assistance to resolve client- physician issues by proactively providing information
- Assists the Sales Representatives with informing physicians on new program initiatives
- Acts as a liaison and provides ongoing feedback to the client based on observations in the field and feedback from customers as it relates to quality of service
- Responds to all physician and patient requests within’ a reasonable time frame outlined in expectations
The Field Case Manager will also be assigned other duties and tasks as required from time to time.
Experience And Educational Requirements
- RN or RPN; Proof of Registration with the Provincial College of Nurses, renewed on an annual basis
- Proof of valid professional liability insurance coverage, renewed on an annual basis
- Minimum three (3) years nursing experience in a clinical setting an asset
- Ability to travel within’ an assigned territory is mandatory
- Valid Driver’s License, current auto insurance policy and access to reliable transportation
- Experience and knowledge in Gastroenterology, Dermatology, and/or Rheumatology an asset
- Knowledge in public or private reimbursement structure and process an asset
- Background in specialty medication/ biologics an asset
- Proficient in computer skills and advanced technology systems.

- Intermediate knowledge of Microsoft Word, Excel, PowerPoint and other Office Programs
- Complies with all legal and regulatory requirements related to healthcare delivery
