Community of Hope is seeking a Referral Specialist to support our dedication to providing exceptional patient care and seamless access to needed medical services. As a Referral Specialist, you will work closely with patients, providers, and insurance providers to ensure that COH patients are receiving the specialty care they need by processing referrals in a complete, accurate, and timely manner, including coordinating with insurance requirements and specialist availability. This position is located at our Family Health and Birth Center in NE, Washington, DC.

What You'll Do

Completes all urgent referrals while the patient is in the office. If this is not possible, the referral will be completed within 1 business day.

Completes all non-urgent referrals within 5 business days of issuance or sooner when possible. Completes pre-authorizations for all diagnostic testing, therapy, surgical and specialty care according to insurance requirements within 3 business days of issuance

Notifies patient via phone when referral is approved. Notifies Provider vie eCW if referral is denied or if a second opinion or additional information is required.

Facilitates patient care process through to completion; this includes but is not limited to scheduling of appointments, arranging interpreters and transportation if necessary.

Maintains up-to-date knowledge of all health care plans. Interprets and communicates plan policies and procedures to employees, physicians and (patients) when they have changed.

Maintains a list of specialists and their requirements to ensure a successful referral process and disseminates this information when appropriate to Providers and other clinical staff so they can provide sufficient information and appropriately request a specialist referral.

Coordinates all aspects of the Catholic Charities referral program, including ensuring that COH and patients are complying with requirements of the program.

Identifies and attempts to resolves patient’s barriers to specialty care and discusses trends with supervisor.

Maintains the diagnostic imaging tracking database and follows up with patients to ensure the procedure was completed. Sends a request for results to the specialist if the patient reports a completed visit.

Refers patients who are experiencing difficulties making or keeping a specialist appointment to appropriate care coordination staff including perinatal or my health GPS (MHGPS) staff as appropriate.

Cross trains in medical records and assists with duties assigned to the Medical Records Specialist as needed.
Requirements:
Must-Haves

High school diploma or equivalent.
- Strong computer skills.

Excellent customer service skills with an ability to work in a fast paced environment.

Proof of required vaccinations is required. This includes, but may not be limited to, Flu. COH will consider requests for reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason, subject to applicable law.
Nice-to-Haves

Minimum two years’ experience working in medical setting required, Community Health Center experience.

Experience using an electronic medical record (EMR) system required. Experience with eClinicalworks (eCW).

Bilingual (Spanish or Amharic).
