JOB SUMMARY: Responsible for developing, coordinating, implementing and evaluating program activities in West Tennessee (primarily Northwestern Tennessee) related to maternal and perinatal health in rural counties. This role focuses on providing direct, personalized care coordination and navigation for pregnant individuals and new mothers during and after pregnancy. Works as part of a larger regional team of multi-sector partner agencies to improve and increase access to care for pregnant women and new mothers during and after pregnancy.
EDUCATION/TRAINING REQUIRED:
- Associate’s degree required, preferably in health, social service, or related field.
- Bachelor’s degree in health, social service, or related field preferred.
RELEVANT EXPERIENCE REQUIRED:
- Minimum of three years’ experience is preferred. Experience in maternal care, case management, or health navigation is highly desirable.
- Must be proficient in the Microsoft Office program or programs and able to learn and become proficient with on-line computer applications and software
- Ability to operate computer and other standard office equipment.
- Ability to understand and compose complex written materials.
- Ability to exercise independent judgment with tact and diplomacy in all situations.
- Must be able to assess needs of pregnant and postpartum clients and respond appropriately.
- Strong ability to integrate knowledge and theory into practice.
- Ability to demonstrate skill in organization, coordination and time management.
- Must be able to provide effective instructions to customers.
- Must be able to communicate verbally in an effective and positive manner with customers.
- Must be able to demonstrate effective communication and negotiating skills with and between individuals and groups of people from all socioeconomic backgrounds, including fellow employees, social services providers, business entities, and students and their families.
- Strong influencing, consultative and coaching skills.
Preferred Qualifications:
- Prior experience in maternal health, reproductive health, women's health, or pediatric care coordination/case management.
- Demonstrated working knowledge of health and social service resources specific to West Tennessee's rural counties (e.g., local health departments, community clinics, domestic violence shelters, and food banks), particularly Northwestern Tennessee.
- Training or certification as a Community Health Worker (CHW), Doula, or similar patient navigation role.
Job Duties:
- Serves as client advocate and liaison between customers and other regional maternal care programs
- Helps customers resolve questions, problems and issues accessing care throughout the perinatal period.
- Collaborates with customers to determine best products or services for their needs.
- Provides technical assistance to customers to promote self-sufficiency or skills.
- Serves as the coordinator for assigned grant-related activities.
- Coordinates events for program activities and recruits other program staff to cover events.
SUPERVISION PROVIDED BY THIS POSITION TITLE:
- There are no supervisory or lead responsibilities assigned to this position.
