Summary
- Better Birth Outcomes – Comprehensive (BBO-C) provides pregnant, and parenting clients with direct, comprehensive, 1:1 nursing assessments and assistance to connect and engage with desired services and understand the importance of a medical home.
- The Nurse Navigator provides a comprehensive nursing assessment and any immediate nursing intervention necessary.
- Navigators provide follow-up to dyads (pregnant and postpartum individuals and their infants), helping to connect families to needed supports available in the community.
- Navigators help the client meet their family’s health and wellness goals and promote the reduction of maternal and infant morbidity and mortality.
Responsibilities

Requirements:
- A Registered Nurse (RN) with an unencumbered license pursuant to the Nurse Practice Act [225 ILCS 65] OR
- An Advanced Practice Registered Nurse (APRN) with an unencumbered license pursuant to the Nurse Practice Act [225 ILCS 65]
