As the Foundations Care Coordinator, you’ll serve in a vital role supporting pregnant and postpartum women experiencing homelessness by providing consistent care coordination, guidance, and encouragement. You’ll help residents build strong foundations for long-term success by assisting with life skills, accessing community resources, and creating individualized plans that promote personal growth and stability.

Position Title: Foundations Care Coordinator
Reports to: Lead Care Coordinator
Location: Fort Wayne, Indiana
Schedule: Full-time; On-call rotation required

Position Purpose:
The Foundations Care Coordinator plays a key role in the daily support and development of residents. This position provides life skills coaching, helps residents meet goals, offers emotional and behavioral support, and maintains documentation to ensure effective care and communication across the program team.
Coordinators are responsible for creating a safe and structured environment, modeling healthy behavior, and connecting women to resources that promote independence and family well-being.
Key Responsibilities:
- Provide one-on-one care coordination and support for residents
- Assist residents in setting and achieving goals related to health, housing, employment, and parenting
- Monitor resident progress and maintain up-to-date documentation
- Support life skills growth including parenting, scheduling, and budgeting
- Respond to behavioral and emotional needs with empathy and consistency
- Collaborate closely with Lead Care Coordinator and other staff to ensure unified support
- Transport residents as needed for appointments and errands
- Participate in on-call rotation for after-hours support
- Uphold and model program rules, expectations, and values
Qualifications:
- Preferred: Bachelor’s Degree in Human Services, Social Work, or related field
- At least 2 years of experience in social work, case management, or similar role
- Valid driver’s license and proof of auto insurance
- Must be able to drive a 12-passenger van safely
- CPR Certification preferred
- Excellent communication and interpersonal skills
- Strong organization, flexibility, and emotional regulation under stress
- Ability to work with individuals from diverse socioeconomic and cultural backgrounds
Physical Demands:
