POSITION SUMMARY
The Obstetrics (OB) Care Navigator provides quality, equitable maternal care by functioning as part of a
multidisciplinary team. The OB Care Navigator serves as a liaison, patient advocate, and the care coordinator for high-risk and underserved obstetric populations referred from the Department of Health
(DOH) and correctional facilities. The OB Care Navigator supports culturally competent care, timely triage, outpatient access, and continuity across Health First’s maternal care continuum.
Primary Accountabilites
- Coordinates referrals and triage for OB patients based on risk level and facility capabilities (e.g., high-risk to Holmes Regional Medical Center).
- Monitors and follows up on care plan completion (prenatal, iron infusions, diagnostics, postpartum).
- Accumulates and reports data on patient barriers, compliance, and care gaps to improve safe and position patience care outcomes.
- Participates in performance improvement and maternal health equity initiatives.
- Assists with scheduling and transportation coordination for outpatient maternal services.
- Supports patients navigate Medicaid, public assistance, or uninsured care options.
- Provides aide for the integration of services between public health, correctional, and hospital systems.
- Educates on translation support and culturally appropriate instruction.
- Serves as the central contact for navigated OB patients and community partners.
- Delivers in-person, virtual-based, and group prenatal education classes, along with reassurance and care coordination.
- Designs patient milestone tracking and follow-up support through the perinatal journey.
- Backs trauma-informed care principles with vulnerable and incarcerated populations.
Work Experience
MINIMUM QUALIFICATIONS
- Education: Associate’s degree in Nursing (ASN) or Nursing Diploma.
- Work Experience: Three (3) years’ experience in Nursing or Care Coordination, specifically in the OB or Maternal Health environment.
- Licensure: Registered Nurse (RN) licensure in the State of Florida or endorsement.
- Certification: American Heart Association Basic Life Support (AHA BLS) Healthcare Provider Completion Card prior to start date and maintained.
- Skills/Knowledge/Abilities:
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other office software.
- Ability to work autonomously with minimal supervision.
- Robust critical thinking skills.
- Strong written and verbal communication skills, with attention to detail and accuracy.
- Excellent time management and multitasking abilities, with the capability to prioritize effectively.
- Ability to maintain composure in stressful office environment.
Preferred Qualifications
- Education: Bachelor’s degree in Nursing (BSN), or Healthcare Administration.
- Skills/Knowledge/Abilities:
- Ability to proficiently speak both the Spanish and English languages.
- Familiarity with Health Department or correctional care systems and with culturally competent or community-based care models.
Physical Requirements
- Majority of time involves sitting or standing; occasional walking, bending, and stooping.
- Long periods of computer time or at workstation.
- Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
- May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
- Communicating with others to exchange information.
- Visual acuity and hand-eye coordination to perform tasks.
- Workspace may vary from open to confined.
- May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
