A dedicated trained doula to serve as a patient care navigator for the Bridgeport Hospital Collaborative Obstetric Medicine clinic and Postpartum Heart Care Program. The Obstetric Medicine Doula will work with patients attending the Collaborative Obstetric Medicine clinic and Postpartum Heart Care Program. While the majority of patients will be postpartum, the Obstetric Medicine doula will also work with select pregnant patients.

1. Educate patients about the Postpartum Healthcare Program
2. Call patients if they have missed appointments
3. Follow-up with patients after ED visits
4. Check in on patients who have mental health needs
5. Remind patients to do labs
6. Review lifestyle [e.g. nutrition] plans .
7. Perform screening for the social determinants of health
8. Connect patient with resources such as transportation, diaper banks, community programs etc.
Qualifications:
EDUCATION
High School Diploma or GED required. Completion of approved DONA International doula training required.
EXPERIENCE
High School Diploma or GED required. Completion of approved DONA International doula training required. Experience with birthing families and prenatal and postnatal care required.
