Position Summary: Despite having one of the most technologically advanced health care systems, the U.S. continues to have high rates of maternal morbidity and mortality. Health inequities contribute to these elevated rates, and thus optimizing maternal health care in the community health setting is important. The OB Nurse Navigator is a professional nurse who serves in a critical role to provide oversight for high risk aspects of antepartum and postpartum care in the OB/GYN department. The OB Nurse Navigator works closely with obstetric providers, primary care clinicians, behavioral health professionals, and community-based services to bridge gaps in care, improve adherence to treatment plans, and enhance patient engagement. The OB Nurse Navigator works directly with the Director of Women’s Health Practice & Projects. Leadership Responsibilities: Performance Improvement monthly, Attend all relevant weekly departmental rounds, Perinatal Health rounds monthly, Collaboration with OB/GYN department nursing team, Collaboration with OB Care Navigator. Responsibilities: Coordinate high-risk OB patient care, providing case management and follow-up for patients facing medical, behavioral health, or social challenges. Serve as a primary liaison between OB/GYN, Family Medicine, and external partners to ensure continuity of care and successful referrals to social services. Use EPIC work-bench reports and the Perinatal Dashboard to create workflows to outreach for referrals. Collaborate with the Maternal Health Equity Council to integrate patient-centered feedback and improve care models. Provide ongoing education to providers, nurses, and care navigators on best practices for addressing maternal health disparities and implementing trauma-informed care. Track patient outcomes, ensuring that those with identified SDOH needs receive timely referrals and complete follow-up care. Job Components: 40 hours will be spent in the above noted responsibilities. Proficiency in EPIC Electronic Medical Record. Resourceful, self-motivated, ability to prioritize, takes initiative. QUALIFICATIONS: Required: Completion of an accredited RN program with a MA License, minimum 3-5 years clinical experience. BSN preferred or in progress. Bilingual preferred. MONDAY - FRIDAY 8:00am - 5:00pm.
