To be responsible for the assessment, planning, implementation and evaluation of evidence based individualised plan of care. The post holder will also assist in the management and organisation of nursing work in the ward/department.

- Recognise any deterioration or change in a patient’s condition which may require the
intervention of others and refer as appropriate.
- Work collaboratively as part of the multidisciplinary team, establishing and maintaining
effective communication and relationships.
- Provide early intervention to ensure the safe discharge of patients, working in collaboration
with other relevant teams and services to prevent unnecessary readmission of patients.
- Act as an advocate for the patient and their family, protecting their interests and statutory
rights, whilst promoting privacy, equality, diversity, and inclusion.
- Maintain a professional image in accordance with the uniform policy and uphold the Trust
values and behaviours, promoting One Culture of Care.
- Direct and supervise the work of non-registered colleagues including, but not limited to,
healthcare support workers and student nurses.
- Undertake additional training to ensure competence to undertake any extended skills or
specialty-specific clinical procedures as required.
- Adhere to the NMC Code and Calderdale & Huddersfield NHS Foundation Trust policies.
