Job Summary
This is an unusual opportunity to join a mature and well developed Primary Care Network (PCN) in an adapted split role. We are seeking a qualified nurse (ideally with some kind of primary care experience) to be employed by West Leeds PCN but mainly deployed at Manor Park Surgery, one of our member practices. The successful applicant would work for 80% of their working week at Manor Park doing the practice work they require; the remaining 20% will be PCN work which will cover all of our 6 member practices. This role will be varied and interesting and will require moving around between all of our member practices. Therefore, a driving licence and access to a car will be crucial. Main duties of the job
The workflow elements of this role will be predominantly as you would expect from a practice based nurse but with the added variety of some PCN work. Please see the uploaded job description for more information about the responsibilities of the role
About Us
West Leeds Primary Care Network (PCN) has a significant workforce of around 60 people in varying clinical disciplines. The PCN already employs several specialist nurses who work at scale on a pro-rated basis across all of our six member practices. Manor Park Surgery, the main base for this role has GP partners, a nursing and HCA team plus a full administrative team on site to help you bed in to this slightly unusual role. Travel vaccinations
Assess, plan, develop, implement, and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
- Implement and evaluate individual treatment plans for patients.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Support patients to adopt health promotion strategies that encourage patients to live healthily and apply principles of self-care.
- Deliver opportunistic health promotion where possible.
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions within your sphere of competence.
- Support patients to adopt health promotion strategies that promote patients to live healthily and encourage principles of self-care.
- Assess and care for patients presenting with wounds and leg ulcers operating within your sphere of competence.
- Support and advise women requesting information relating to family planning needs.
