Job Purpose/Summary: The OB Care Coordinator (OBCC) works under the direction of the Referral Manager and supports SBCHC healthcare professionals to provide coordination of care to all SBCHC patients. The OBCC is responsible for assuring continuity of care for those patients within the Obstetrics department of patient care. The OBCC works as a liaison between the OBGYN care provider, insurance companies, specialty providers, diagnostic services, and patients. The OBCC also assists in supporting a safe and clean environment, and acknowledges SBCHC’s Mission and Vision. The OBCC will work directly with vulnerable populations including but not limited to Communities of Color, Women with Children, Pediatrics, Youth, Individuals and Families Experiencing Homelessness, People Living with HIV/AIDS, and individuals with mental health and/or substance use disorders (SUDs) including co-occurring issues.
Policy: It is the policy of SBCHC that the Referral Department performs duties that are within limitations of the scope of practice for Community Health Workers. The OB Care Coordinator must perform duties in line with the Health Insurance Portability and Accountability Act (HIPAA). It is the responsibility of the Referral Manager and OB Care Coordinator to know their limitations and abide by federal and state regulations.
Essential Duties & Responsibilities:
- Conducts initial outreach to OB patients to provide an overview of pregnancy, visits, imaging, labs, and other patient related care.
- Completes initial OB intakes to provide extensive history for provider care, and documents findings within electronic health record.
- Provides information to patients in regard to insurance carriers, or patient programs that assist with cost of services.
- Demonstrates ongoing adherence monitoring prenatal, antenatal, postpartum, and newborn visits to ensure continuity of care, scheduling patients as needed throughout pregnancy.
- Completes follow up calls to patients throughout pregnancy to ensure compliance to plan of cares, addressing and communicating any additional needs with OBGYN provider directly.
- Completes transfer of care specific to patient demographics, insurance, and delivery needs to each OB patient. Provides information directly to OB patient and documents in electronic health record.
- Follows up with OB patients who are set to deliver to schedule postpartum visit and newborn visit with SBCHC.
- Collaboration with hospital systems to ensure delivery of care is provided to each OB patient fluidly.
- Initiate the completion of all forms for review and final signature by the provider (disability forms, home health orders, durable medical equipment and supply requests, etc).
- Maintains OB Census accuracy to ensure compliance to UDS reporting needs.
- Promotes OB Classes to patient for additional resources to OB patients.
- Process referrals according to the level of urgency in a timely manner.
- Entry of all referral information into electronic health record or other referral module management systems.
- Submits and obtains authorizations to insurance, tracks the status of authorizations and once approved, documents procedure approval number and authorization period in electronic health record or other referral module management system.
- Assists referral department with non-OB specialty referrals based on OB census.
- Seeks assistance of supervisor when unable to process referrals in a timely manner to avoid backlog and delays in care for patients.
- Upholds strict patient confidentiality requirements required by federal and state law.
- Actively provide willingness to help and maintain professional relationships with coworkers.
- Perform other duties as assigned, within scope of practice.
- Attend all staff meetings and mandatory In Service meetings.
- Observe and acknowledge all SBCHC policies and procedures, including all emergency and disaster response responsibilities.
- Participates in Compliance, Risk and Safety, and Quality Assurance and Performance Improvement initiatives.
- Always greet patients, visitors, and clinical staff in a courteous and respectful manner; respond to patients with empathy and positive interpersonal skills; consistently handle all requests in a positive manner.
Qualifications:
- High School Diploma or GED
- Bachelors degree preferred
- State of Texas Community Health Worker certification required
- Two years of experience within healthcare or health insurance field preferred
- Current Basic Life Support (BLS) or BLS equivalent
- Multi-lingual, English/Spanish preferred
- Available for all shifts/assignments and, when required, able to work evenings and weekends
