Job Summary: Under limited supervision, uses intermediate skills obtained through experience and training, to provide support to expectant and postpartum mothers. Employs various activities and acts as a liaison to LAHC’s Maternal & Infant Health Resource Hub, community service, and other resources to ensure families receive timely access to pre- and post-natal care, supports, and other engagement opportunities. Routine contact with internal and external stakeholders, including local organizations, schools, healthcare providers, and community leaders is required to meet goals and objectives.
Essential Duties and Responsibilities:

Conduct comprehensive intake assessments and create individualized care plans to identify maternal health needs.

Work closely with individuals and families to provide guidance, information, and resources to support maternal health options.

In collaboration with community advocates, outreach staff, and health care providers, raise awareness of the need for maternal health services.

Support facilitation of pre and postpartum peer support groups (in-person and virtual).
-
Develop and attend community events to promote awareness of the benefits of maternal health and the importance of healthy options.

Provide ongoing support to mothers throughout pregnancy and up to six months postpartum, including regular check-ins and assistance with navigating health care, social services, and community resources.

Assist clients in overcoming barriers to care by offering referrals, appointment scheduling support, and follow-up to ensure continuity of services during the prenatal and postpartum periods.

Develop collaborative efforts with community organizations.

Distribute outreach and other materials to expectant moms, community members, partner organizations and businesses to build awareness.

Assist in the development and implementation of organizational outreach and enrollment initiatives.

Participate in various task forces or committees addressing program needs.

Participate in health fairs, and other community projects with the goal of increasing awareness of our programs and services.

Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.

Accurately provide required reporting to track goal achievement and client satisfaction.

Maintains regular and consistent attendance.

Operates standard office equipment and uses the required software applications.

Educational/Previous Experience Requirements:

Minimum Degree Required: Associates degree (bachelor’s degree preferred)

Required Disciplines: Public Health, Social Work, or a related field or Minimum one (1) year of experience in a community facing role, addressing maternal health or any equivalent combination of experience, education, and/or training approved by Human Resources. Bilingual in English and Arabic required.

Certification in Community Health Work (CHW) or willingness to obtain within 6 months.
Working Conditions:
Hours: Normal business hours. Some additional hours or ability to flex hours may be required.
Travel Required: Local travel within the Detroit metropolitan area.
Working Environment: Climate controlled schools, community centers, health clinics and office environment during normal business hours.
