Job Title: Integrated Care Nurse Coordinator
Department: Home and Community Care Program
Pay Range: $36.55 – 45.01 per hour

Position:
The Integrated Care Nurse Coordinator will be responsible for the delivery and coordination of comprehensive, culturally sensitive, community-based health services across the life span, with a focus on high-needs individuals and families. This role merges clinical case management with program coordination, ensuring quality care, staff supervision, and administrative excellence within the Home and Community Care Program. The position reflects the philosophy and policies of Walpole Island First Nation, adhering to the standards set by the College of Nurses of Ontario, the RNAO, and government regulations. The successful candidate will work collaboratively with interdisciplinary teams to enhance health outcomes, promote independence, and foster a supportive care environment for clients of all ages.
Requirements:
A. EDUCATION
- BScN or Registered Nurse with specialized training (i.e., Maternal Child Health, Community Health, or equivalent)
- Current Registration in good standing with the College of Nurses of Ontario.
- Current CPR and First Aid Certification (or willingness to obtain).
- Crisis Intervention and Trauma & Loss in Children Training (or willingness to obtain).
- Office Administration or PSW Certificate is an asset.
B. EXPERIENCE
- Minimum 3 years’ clinical nursing experience in community health, long-term care, or case management.
- Minimum 2 years’ supervisory or program coordination experience.
- Experience working with First Nations Communities and Indigenous Health Programs is preferred.
C. SKILLS & ABILITIES
- Strong case management, leadership, and conflict resolution skills.
- Excellent interpersonal, communication, and organization skills.
- Knowledge of health promotion, chronic disease management, and mental health supports.
- Proficient in computer use and electronic health documentation.
- Respect for confidentiality and professional boundaries.
D. OTHER
- Valid Driver’s License and reliable transportation.
- Must pass a Vulnerable Sector Criminal Record Check.
- Willingness to work flexible hours, weekends, holidays, and on-call shifts
- Ability to travel as required.

Responsibilities:
Clinical Case Management:
- Conduct holistic assessments and develop individualized care plans for clients from birth through to elder care.
- Provide continuity of care, supporting transitions between hospital, home, and long-term care.
- Advocate for clients and coordinate access to internal and external resources and funding programs.
- Deliver trauma-informed, culturally appropriate, and client-centered care.
Program Coordination & Supervision:
- Supervise and support Personal/Home Support Workers, including scheduling, performance feedback, and training needs identification.
- Collaborate on PSW evaluations and assist in pay-for-performance documentation.
- Maintain client satisfaction through regular check-ins, service evaluations, and resolution of concerns.
Administration & Reporting:
- Submit reports, billing claims, and statistical data to funders and the Finance Department.
- Reconcile financial reimbursements and maintain accurate records in alignment with internal policies.
- Oversee scheduling, payroll preparation, and maintain up-to-date client service documentation.
Team Collaboration:
- Act as a clinical resource and mentor within the interdisciplinary care team.
- Participate in plan-of-care meetings and regular staff meetings.
- Assist with onboarding, cross-training, and development of internal procedures and initiatives.
Community Engagement & Advocacy:
- Support community education through outreach and health promotion.
- Identify community health trends and contribute to program planning and evaluation.
- Ensure services are delivered in alignment with community values, rights, and needs.
Other:
- Other duties and responsibilities as assigned by supervisor.
