Duties

- Provide direct patient care in acute care settings, ensuring comfort and safety.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Conduct assessments and document patient health status using electronic health record systems such as Epic.
- Educate patients and families on health management, including dementia care strategies.
- Maintain accurate medical records and ensure compliance with healthcare regulations.
- Participate in case management activities to coordinate patient services effectively.
- Support patients through various stages of recovery, providing emotional support as needed.
Skills
- Strong knowledge of medication administration practices.
- Familiarity with medical terminology relevant to nursing practices.
- Experience in acute care environments or home care settings is preferred.
- Proficiency in using electronic health record systems, particularly Epic.
- Excellent communication skills for effective interaction with patients, families, and healthcare professionals.
- Ability to manage multiple tasks while maintaining attention to detail.
- Compassionate approach towards patient care, especially for those with dementia or chronic conditions.
- Strong organizational skills for managing case management responsibilities.

Pay: $27.00-$51.15 per hour
