Overview:
We are seeking a dedicated and compassionate Case Coordinator to join our team. This is a critical position at Mama’s House. It ensures that efficient routines are maintained while the individualized Pathway To Success program for each resident is instigated and managed accordingly.
The ideal candidate is a Christian who loves the Lord and deeply desires to serve Him in this ministry. Possesses a servant’s heart, full of love, compassion and kindness as a role model for the residents. Knows how to work independently and as a team member. With excellent communication, interpersonal, problem solving and organizational skills. Understands priorities and completes tasks on time, highly dependable and trustworthy. Maintains and respects confidentiality and privacy in all matters pertaining to the residents.
Hours:
Monday -Friday 8 am to 4:30 pm / 40 hour week
Responsibilities of Case Management:
- Meet weekly to review the calendar of each resident. Ensure residents are on track with their individual “Pathway to Success” program. Setting up treatment options and services, such as arranging transportation and inputting appointments into Volunteer Scheduler Pro (VSP) and requesting necessary volunteers such as drivers or babysitters.
- Request resident transcript from last school. Gather information of past work or volunteer experience to establish a plan. The goal is to have the residents be in school to earn their GED/diploma or higher education. Residents will work or be enrolled in internships or volunteer work.
- Process background checks in addition to Riverside County and other agencies.
- Earn while you Learn (E&L) track progress with residents must complete 100 hours of curriculum.
- Assist with setting up doctor, dental, WIC, and any other related appointments.
- Ensure residents are enrolled for Mama’s House classes such as Step Study, Domestic Violence, tutoring and completion is tracked.
- Schedule individual therapy with counselor and mentor appointments.
- Our residents are required to save 85% of their income into a savings account. Taking bank deposits and tracking expenses. Receive cash requests from residents and work on a monthly budget worksheet.
- Consult and collaborate with other health care providers and specialists to set up resident appointments and treatment plans for medications.
- Processing a credit check to improve credit score by paying off debt or setting up
payment agreements. Get clearance of “no evictions” from past renters.
- CORP- Represent Mama’s House at meetings. Complete and track progress of resident’s application and their requirements to clear fines and fees in Riverside County.
- CPS- Point of contact for any open cases with residents. Ensure residents are participating in required classes outlined by social workers. Attend any court dates as required.
Duties:
- Oversee the daily compliance of residents schedule at Mama’s House
- Responsible for the welfare and well-being of residents within the house and aim to provide a caring, safe and harmonious environment
- Maintain written daily communication report on each resident
- Assume the role of a maternal influence
- Assist with social correctness, etiquette, and professional courtesies
- Respond to emergencies and call for medical assistance as needed
- Implement and assure policies and guidelines are followed in the Resident Handbook
- Interface and resolve conflict between residents
- Ensure that individual needs of each resident are communicated with staff in a timely manner
- Encourage the “Pathway to Success” with each resident
- Understand the importance of observation and contact with all the residents
- Report to Program Director
Skills and Abilities:
- Must be a good listener, non-judgmental, have compassion and empathy
- Must be proficient in computer skills and operating office equipment, computer, fax and copier
- Must be physically sound as position entails; sitting, walking, and standing throughout the shift and may be required to lift, push, and pull up to 25 pounds
- Strong analytical thinking and ability to handle multiple tasks concurrently
- Good verbal and written communication skills
- Must be comfortable working with babies, basic knowledge of childcare
Requirements:
- High School Graduate or higher
- Social Services experience
- Must have a valid Driver’s License and insurance
- Certifications - CPR and First Aid
- Computer Literate
- Drug Free
