The Behavioral Health Care Coordinator position primarily involves in-person case management. The case management portion of this role entails providing direct support to patients referred to the Behavioral Health Care program or identified through on-site care. The Coordinator may manage a small caseload of psychotherapy patients, with a primary emphasis on those with significant social needs.

CASE MANAGEMENT:
- Engages in daily behavioral health team functions to support consistent and timely responses to patient needs including, but not limited to, daily huddles, review and response to universal patient screening related to social determinants of health, and consultation with medical providers requesting input on patient care.
- Offers on-site clinic engagement and assistance for same-day patients, particularly those recognized as having high-risk factors or complex social requirements.
- Serves as the primary contact for responding to inquiries received through the Behavioral Health (BH) backline phone line; collaborates with the Behavioral Health Clinician (BHC) on duty and the on-call Behavioral Health manager to address requests from staff, providers, and patients.
- Completes case management tasks such as referrals to home visiting and specialty mental health programs, and provides detailed and skilled resource referrals in response to Behavioral Health Clinician (BHC) and medical provider requests. Documents and provides follow up and communication to providers and BHCs.
- Coordinates and collaborates with medical providers, staff, families, and community providers. Maintains chart notes related to internal and external contracts, obtains pertinent records from other settings and supports patient’s involvement, follow-through, and progress in varied treatment activities.
- Takes primary responsibility for supporting consistent outreach efforts to high needs patients not engaged in therapeutic care with primary goal of building trust and rapport with patients in order to facilitate connections to accepted and beneficial support in the community or within WHA.
- Coordinates and facilitates complex case review meetings in conjunction with licensed team members in order to promote high-quality team-based care and support for patients.
- Effectively manages crisis situations involving patients who may be distressed or experiencing a mental health crisis by maintaining composure and facilitating de-escalation. Conducts thorough assessments of patient safety and identifies suitable subsequent actions to ensure stabilization and coordination of care. Employs the CSSRS screening tool as part of the assessment process.
- Takes on special projects focused on enhancing care management, as delegated.
- Provides timely and succinct feedback to other clinicians regarding consultation findings, recommendations, referral updates, and other case management needs.
- Participates in regular case consultations and one on one supervision opportunities and seeks opportunities to contribute to team culture.
- Works as part of a multi-disciplinary team in making recommendations for clients who abuse, misuse, or divert controlled substances.
- Minimum CSWA, Registered Associate LPC or LMFT within 1 month of start date. Must be registered and under plan of supervision for work hours to count toward licensure. Licensure supervision may be included depending on current staffing. Fully licensed applicants also welcomed.
- Ability to obtain NPI number within 1 month of start date.
- Ability to credential with insurance plans that accept CSWA, Registered Associate LPC or LMFT practitioners within 1 month of start date. Fully licensed practitioners eligible for credentialing also welcomed.
- Insurable by malpractice carrier for WHA in minimum amounts required by WHA.
