Position Overview
The Clinical Nurse Coordinator reports to the Medical Director. This position will provide strategy, judgment, organization and evidence-based analysis to influence decisions, and directly to meet Care Medical Practice’s requirements. The Clinical Nurse Coordinator should embody Care Medical’s core values, including Humility, Partnership, Transparency, Creativity, Integrity and Empathy.
Key Responsibilities
The Clinical Nurse Coordinator will have the following responsibilities:
- Prepare and complete prior authorizations for medications and diagnostic testing utilizing insurance and formularies.
- Assistance with chart preparation, specifically scheduling and follow through of medical clearance appointments.
- Prepare patients for provider examinations including, not limited to vitals, BMI, blood pressure, height, weight, and pulse.
- Conducting point of care testing, including but not limited to glucose finger sticks, venipuncture, EKG, pregnancy tests, Covid testing and urine testing.
- Administer immunizations, medications, and injections as appropriate. Perform dressing changes and wound care as appropriate.
- Manage clinical supplies and inventory, checking expirations, and stocking rooms as indicated.
- Monitor clinical logs including not limited to medication and refrigerators logs to ensure within standards and procedures within internal and legal regulations.
- Monitor sample logs and appropriate disposal procedures within internal and legal regulations.
- Assist with patient phone calls and provide guidance and instruction under the direction of healthcare provider.
- Completion of forms as indicated and scheduling of patients.
- Provide patient education under the direction of Registered Nurse or Provider including but not limited to diabetes education, discharge instructions and medications.
- Monitor, assist and coordinate with providers, managers, medical assistants that quality metrics are complete at time of visit, includes both upstate and downstate.
- Coordination of VFC program both upstate and downstate NY, accuracy of ordering and inventory of supplies
- Responsible and lead on care coordination, transition of care and emergency department follow up, included upstate and downstate practices.
- Assist with identifying challenges, barriers and providing solutions for improvement and implementation.
- Performance of duties assigned by management as needed.

- Leadership: The Clinical Nurse Coordinator will lead in defining and executing strategies and solutions to create business value in the area of expertise, including building a team to design, develop, and execute those strategies and solutions to deliver desired outcomes.
- Strategy: The Clinical Nurse Coordinator will suggest new programs, procedures and policies to continually improve efficiency and morale and to improve the employee experience.
- Collaboration: The Clinical Nurse Coordinator will work together as an inter-professional, interdependent team in patients’ best interests to support comprehensive care delivery.
- Knowledge: The Clinical Nurse Coordinator will stay up to date on the latest clinical and practice guidelines.
- Culture: The Clinical Nurse Coordinator will maintain a productive, collaborative, safe and inclusive work environment among the entire Care Medical Practice community
Required Qualifications
- Education:
- Associates as a Licensed Practical Nurse from an accredited facility.
- Current license to practice as a Licensed Practical Nurse in New York State.
- Cardiopulmonary resuscitation certification (CPR) preferred.
- Experience:
- Clinical and administrative nursing experience practicing in a value-based clinical model.
- Experience in an urban setting preferred.
- Minimum of 1 year experience preferred.
- Communication:
- Excellent verbal, written communication, and presentation skills; Exceptional interpersonal and communication skills Proficiency in all Microsoft Office applications.
- Bilingual communication skills (preferred).

- Ability to build and effectively manage relationships with business leaders, coworkers, and clients.

The working environment and physical requirements of the job include:
In-office work (
In-office work. The job is performed indoors in a traditional office setting with air conditioning, artificial light, and an open workspace.
In this position you will need to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculators, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
Compensation: From $58,458 to $60,000 per year
