Program Description
Under the supervision of the Queens Healthy Start Program Manager, the Community Action Coordinator (CAC) will be the primary lead for the Community Consortium under the maternal and child health program, Queens Healthy Start (QHS). QHS works to improve health and wellness throughout the reproductive life course, with a focus on Black/African American pregnant women and their families, and to reduce inequities in maternal and infant morbidity and mortality. Queens Healthy Start is focused on South Queens neighborhoods, including Far Rockaway, where disparities in birth outcomes are greatest.
Summary Of Position
The QHS Community Consortium, Queens Alliance for Thriving Families (QATF), is a group of community members, providers, elected officials, and other stakeholders, that meet regularly to discuss ways to address the issues that lead to poor infant and maternal outcomes in the community, and create a collective impact. The Community Action Coordinator will lead the QATF meetings, establish new partnerships in the community and facilitate the implementation of a shared agenda created by the collaborative partners of the Community Consortium. They will lead the group in developing an annual strategic plan by gathering and analyzing data to help identify community health needs and bringing awareness and action to the elimination of disparities in birth outcomes for pregnant women of color in the QHS service area.
Specifically The, Community Action Coordinator Will
- Lead/facilitate bi-monthly Queens Alliance for Thriving Families (QATF) meetings, virtually or in person.
- Facilitate the executive team meetings in collaboration with the QATF executive team members.
- Conduct continuous targeted outreach to community partners and local community groups, agencies, and organizations to identify partners and build a cross-sector Consortium.
- Work with partners to identify, recruit, and retain community residents to participate in the Consortium.
- Coordinate training for Consortium members in skills such as community-based participatory research, data analysis, and asset mapping and on topics such as toxic stress, trauma-informed care, and health insurance enrollment and others QATF has selected.
- Provide guidance and lead QATF in selecting a common agenda, shared measures, and mutually reinforcing activities.
- Represent QHS and QATF at outreach events and stakeholder meetings.
- Plan and organize events such as focus groups, town halls, health, and educational sessions, and outreach events.
- Work with Public Health Solutions evaluation team to design and implement evaluation activities for the Consortium.
- Draft reports regarding grant activities and program evaluation and outcomes.
- Other responsibilities as assigned by the Queens Healthy Start Program Manager.
Qualification And Requirements
- Minimum Associate’s degree or equivalent.
- 1+ years of experience in community organizing, strategic planning, and supporting cross-sector partnerships.
- Passion for birth justice and disparities in maternal and infant health
- Experience in public health and community development in urban, underserved neighborhoods
- Bi-lingual preferred (Spanish or Haitian Creole), but monolingual English speakers are also encouraged to apply.
- Outgoing personality, comfortable engaging community members and elected officials to advance the goals of Queens Healthy Start and comfortable with public speaking.
- Ability to be non-judgmental and establish trusting relationships; good problem solving, listening and practical skills; demonstrates maturity, self-awareness, and emotional well-being.
- Shows respect and sensitivity with individuals of differing cultural, racial, sexual, or ethnic identities.
- Great attention to detail.
- Open to working evening hours and weekends when necessary.
- Demonstrated ability to work well independently and stay organized as well as work as a contributing member of a team.
