Description
OB Outreach home nurse serves as a link between the health system and community resources for vulnerable populations. Provides comprehensive clinical nursing support to mothers and babies after discharge from SJHS Mother/Baby unit. The position will be collecting and documenting vitals, medical history, allergies and current medications in homes; as well as providing resources, referrals, and education to the patient tailored to their unique socioeconomic status and living circumstances. Provide post-partum assessment for mom and infant, breastfeeding assistance, depression screening, and emotional support. Follow-up with patients in need of short-term case management in order to obtain safe sleep, car seats, specialty care, or basic needs.

About The Job
As a nurse at Saint Joseph Health System you are responsible for implementing, directing, communicating, and documenting the nursing process in accordance with current established standards of care and the Indiana Nurse Practice Act. Our RNs are patient advocates who provide a safe environment for patients, visitors, and colleagues.
Requirements
- Education: Graduate of an accredited RN program.
- Licensure: RN Indiana License. CPR certification on hire or able to attain within 60 days.
- Experience: None.
