JOB TITLE: Nurse Practitioner/Physician Assistant – Primary Care
SCHEDULE: Full-Time, 40 hours/week,
Monday -Thursday 8am-5pm (30 minute lunch)
Friday's 8am -2:30pm (30 minute lunch)

Summary Of Job Posting
Provides general medical care and treatment to patients in the Coastal Horizons Center clinic, under the direction of the Primary Care Medical Director. Nurse Practitioner or Physician’s Assistant provides diagnosis, treatment, consultation, and follow-up medical care for patients including those with chronic and complex health needs. The Nurse Practitioner or Physician’s Assistant provides age and specialty appropriate medical care at the level of training achieved.
Required Educational/ Qualifications
- Licensed in the state of North Carolina as either a Nurse Practitioner or Physician’s Assistant
Position Highlights
- Conducts physical examinations, takes medial history and diagnoses health conditions.
- Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures under the supervision of the Medical Director.
- Orders, interprets, and evaluates diagnostic tests, including labs and imaging, to identify and assess patient's clinical and health care needs.
- Records physical findings in electronic health record and formulates plan and prognosis, based on patient's condition.
- Staff cases with supervising physician and other healthcare professionals to ensure coordinated and effective patient care.
- Refer patients to physicians for consultation and/or to specialized health resources for treatment.
- Provides patient education regarding chronic and complex health needs.
- Counsel patients on treatment options, lifestyle changes, and preventive measures.
- Performs ongoing medical care management assessment, referral, and evaluation of the life domain needs of patients. Provides information and collateral contacts with needed parties.
- Develops, maintains and supports a network of agency contacts and resources to assist patients with medical needs.
- Works with and provides direction so medical support staff.
- Participates in weekly Interdisciplinary Meeting to ensure whole person care integrated care.
- Participates in providing life skills, crisis contingency planning, disease management, treatment support activities adapted or specifically designed for persons with physical disabilities or persons with co-occurring disorders or mental illness and substance abuse/dependence or mental retardation/developmental disability and substance abuse/dependence.
- Supervise intern(s) when appropriate with understanding program operations, reviews documentation, and directs client interaction.
- Subject to inside environmental conditions and exposed to fumes, odors, dusts, mists, gasses or poor ventilation and subject to work in close quarters. Employees may be exposed to human body fluids and are covered by the OSHA rules regulations on blood borne pathogens.
Additional Information
- Meets requirements under the State of North Carolina as a Licensed Nurse Practitioner or Physician’s Assistant
- Strong clinical knowledge and solid understanding of medicine, anatomy, pharmacology, and diagnostics.
- Strong and clear communication for explaining diagnoses and treatments to patients, collaborating with healthcare teams, and documenting medical information accurately.
- Strong critical thinking skills and analytic skills with the ability to make decisions in high-pressure situations.
- Demonstrate empathy and strong interpersonal skills that foster compassionate care and build a trusting, strong patient-provider relationship.
- Attention to detail and precision in diagnosing, treating, and following medical protocols to ensure the best patient outcomes.
- Ability to be flexible and manage changes in patient conditions, schedules, and medical practices policies and procedures.
- Proficient in using medical tools, technology, and electronic health record (EHR) systems.
- Ability to establish and maintain effective working relationships with co-workers, supervisors, and members of civic, medical, social, and religious organizations.
- Must be able to physically perform the basic life operational functions of stooping, reaching, and walking, fingering, grasping, talking, hearing, and performing repetitive motions.
- Must be able to perform sedentary work exerting up to 10 pounds of force occasionally and/or a negligible amount of force constantly or frequently to move objects.
- Must possess the visual acuity to perform extensive reading.
- Must possess a valid NC Driver’s License.
