Position Summary
The LPN Intake Resource Nurse serves as a key point of contact for new patient admissions and ensures a seamless transition for individuals receiving home health care services. This role involves assessing patient needs, collaborating with multidisciplinary teams, and coordinating intake processes to ensure timely and efficient delivery of care.
Key Responsibilities
- Patient Intake and Assessment:
- Conduct initial referral assessments and review medical records to identify specific care needs.
- Collaborate with physicians, case managers, and other healthcare professionals to evaluate care requirements and establish patient care plans.
- Coordination and Communication:
- Ensure all required documentation is completed accurately and submitted on time.
- Communicate effectively with patients and families to explain services, answer questions, and provide guidance on expectations for home health care.
- Ensure compliance with company policies, state and federal regulations, and patient confidentiality requirements.
- Education and Support:
- Provide education to patients and families regarding home care services, medications, and procedures.
- Address patient concerns, offer solutions, and escalate issues to the appropriate team members when needed.
- Quality Control and Documentation:
- Maintain accurate and up-to-date records of patient intake processes, assessments, and care plans.
- Review and monitor patient progress in collaboration with other healthcare team members to ensure effective treatment delivery.
- Collaboration with Multidisciplinary Team:
- Work closely with case managers, registered nurses (RNs), therapists, and physicians to ensure the development and implementation of a comprehensive patient care plan.
- Participate in team meetings to discuss patient care and progress, and adjust plans as needed.
Qualifications
- Licensure: Current Licensed Practical Nurse (LPN) license in good standing.
- Experience: Minimum of 2 years of experience in home health care or a clinical setting preferred.
- Skills:
- Excellent communication and interpersonal skills.
- Strong organizational skills and attention to detail.
- Ability to work independently and as part of a team.
- Proficiency with electronic health records (EHR) and other healthcare management software.
- Knowledge of home health care regulations and compliance standards.
