Integrated Nurse Case Manager/RN/OBGYN - Central/Richmond (Remote)
Job Overview
We are seeking a dedicated and experienced Registered Nurse with a strong commitment to maternal health to join our team as an Integrated Nurse Case Manager. This pivotal role provides an exceptional opportunity to positively impact the lives of pregnant and postpartum women and their families within the Central/Richmond, VA area.
Key Responsibilities
- Develop and execute individualized care plans addressing the diverse physical, behavioral, social, and long-term care needs of pregnant and postpartum women.
- Conduct both telephonic and in-person clinical assessments to evaluate and manage member requirements.
- Coordinate comprehensive care for high-risk members, particularly those with chronic conditions or disabilities.
- Optimize health benefits utilization while managing care effectively, including addressing gaps in care for quality improvement initiatives.
- Facilitate the implementation of care plans through the authorizations and referrals process.
- Collaborate with Medical Directors, Physician Advisors, and interdisciplinary teams to review and discuss treatment plans.
- Maintain compliance with regulations, accreditation standards, and company policies.
- Address and resolve any issues related to providers, claims, or service delivery.
- Provide specialized maternity care and support, focusing on labor and delivery, postpartum care, and mother/baby care.
Required Skills
- Active Registered Nurse (RN) license, with a Compact/Multi-State License.
- Bachelor of Science in Nursing (BSN) is preferred.
- Experience in nursing with a focus on case management, care coordination, or patient advocacy, particularly related to pregnant and postpartum women’s medical and clinical needs.
- Comprehensive understanding of the physical, psychological, socio-cultural, and cognitive requirements of pregnant and postpartum clients.
- Exceptional communication, problem-solving, and analytical skills.
- Ability to work effectively both independently and collaboratively in a remote setting.
Qualifications
- Active RN license is mandatory.
- Preference for candidates with a BSN degree.
- Relevant nursing experience, particularly in maternity care (Labor & Delivery, Postpartum, Mother/Baby), is highly desirable.
