We are seeking a skilled and compassionate certified Nurse Practitioner to join our provider network. In this full-time role, you will conduct in-home wellness risk adjustment assessments for Medicare members and at-risk populations, focusing on their overall health and well-being to ensure accurate risk adjustment coding.
- Perform annual in-home wellness visits and comprehensive health assessments.
- Deliver patient health education and build trust through exceptional communication.
- Assist in closing quality care gaps and collaborate to enhance care delivery.
- Travel within a 55-mile radius for member visits.
