Reporting to the Vice President of Clinical Operations, this leadership role is pivotal in driving care transformation initiatives, overseeing clinical care management, and managing clinical value-based agreements and at-risk contracts.
The Director will spearhead efforts to integrate care management best practices with client operations, utilize data-driven insights for strategic decision-making, and ensure clients achieve optimal outcomes. This will be accomplished through the implementation of efficient, innovative care delivery models that align with value-based care principles. The ideal candidate can work independently with the ability to prioritize and escalate issues as necessary. The candidate must also be able to work comfortably within a fast paced and dynamic work environment, with a high level of autonomy, and ability to embrace change
Responsibilities
- Oversee the implementation of value-based care models, ensuring alignment with evidence-based practices and regulatory standards.
- Collaborate with cross-functional teams (including IT, analytics, and clinical staff) to implement care management protocols, workflows, and technologies that enhance patient outcomes.
- Provide clinical guidance and oversight for all transformation projects, ensuring initiatives align with best medical practices and organizational goals.
- Monitor clinical performance metrics, such as patient outcomes, quality indicators, and utilization data, to assess and improve clinical programs.
- Serve as a clinical expert to address challenges or barriers to transformation, leveraging data to recommend actionable improvements.
- Lead the strategic planning and execution of clinical components in value-based care agreements and at-risk contracts with payers and providers.
- Monitor the performance of clinical programs under value-based contracts to ensure financial and quality targets are met.
- Act as the primary clinical liaison between VBC/risk agreement clients and the organization, ensuring seamless communication, understanding of client needs.
- Participate in regular client reviews to discuss performance outcomes, identify opportunities for improvement, and provide strategic recommendations for optimization.
Qualifications
- Bachelor’s degree or higher in healthcare administration, nursing, medicine, public health, or a related field
- Clinical licensure (RN, NP, PA) strongly preferred.
- Minimum of 7-10 years of experience in clinical operations, healthcare consulting, or transformation leadership, with at least 5 years in a leadership role.
- Experience with value-based care models, clinical quality improvement, and healthcare data analytics
- Proven track record of successfully managing client relationships and improving clinical outcomes through transformation initiatives
- Experience with negotiating and managing clinical value-based agreements and at-risk contracts.
