JOB SUMMARY: Health Educator is responsible for outreach, recruitment, and providing prevention workshops, with a focus on adult women identified as high-risk for STI/HCV, HIV+, and substance misuse. The Health Educator will deliver evidence-based interventions such as Parenting Journey, and WILLOW in a group setting. In addition, work in a team model to support individual sessions and referral follow-ups for Navigation Services.

This role requires up to 75% of traveling and street outreach. This role will require some weekends and evenings.

EDUCATION AND QUALIFICATIONS:

MINIMUM: High School Diploma/ GED with at least three (2) years of professional experience in the field of working with adult women
Bachelor’s degree in health education or related field and one (1) year of experience
MINIMUM: For High School Diploma/GED Candidates: Two (2) years’ experience working in substance use group facilitation with the target population
Experience with conducting , client recruitment, and networking which includes prioritizing population of focus.
Up to 75% of traveling required.

LICENSURE:

A NYS Driver’s License with at least a two (2) year clean driving record, preferred

DUTIES AND RESPONSIBILTIES:

Program Delivery & Interventions

Deliver evidence-based practice (EBP) interventions to fidelity, conducting all phases as outlined in the following curriculum:Deliver high-impact, evidence-based interventions, including: Parenting Journey and WILLOW) in group settings.
Conduct all phases of the program curriculum as outlined.
Ensure the program services and deliverables meet contract requirements.

Partnerships & Outreach:

Create linkages and partnerships with other providers to recruit the target population.
Attend outreach events, health fairs, and tabling opportunities to recruit eligible participants for program enrollment
Attend monthly outreach meetings and participate in evening or weekend events as necessary.
Create engaging content for social media platforms to reach the target audience.
Post at least one piece of content per month and aim for a minimum engagement of five people per post.

Administrative & Reporting:

Maintain and update Outlook and eCR calendars regularly.
Ensure all deliverables are met as stipulated in the program contract.
Maintain updated and accurate individual records for all clients via eCR.
Complete monthly reports and close client charts according to BAC and department standards.

Meetings & Collaboration

Attend the monthly Outreach and Partnership meeting.
Participate in Continuous Quality Improvement (CQI) projects.
Attending agency committee meetings, supervision, team, and department meetings.
Collaborate effectively with team members and complete additional tasks assigned by the supervisor.

Professional Development & Compliance

Attend relevant training and conferences pertaining to the program and target audience.
Join community committees, task forces, or networks related to the program and/or target audience.
Adhere to the business casual dress code.
Perform other duties as assigned based on the needs of the program.

KNOWLEDGE, SKILLS, AND ABILITIES:

Ability to work with multiple targeted populations such as adult women, HIV+, high-risk/at-risk individuals.
Ability to conduct HIV Testing and Hepatitis C, a plus
Previous experience delivering evidence-based interventions and or group facilitation
Ability to withstand extended sitting, extended standing, stairs, and extended travel with or without reasonable accommodations.
Must be willing to work weekends, nights, and long/flexible hours as required by the position.
Outstanding writing and communication skills; including Twitter/Facebook and other multi-media outlets.
A NYS Driver’s License in good standing, a plus
Strong outreach Skills, including creating partnerships,
Familiar with using EHR systems, plus.
Effective communication, including writing, speaking, active listening, and presenting.
Up to 75% of traveling required.

Job Type: Full-time

Pay: $24.27 per hour

Expected hours: 35 per week
