What You’ll Do

Conduct thorough patient admissions, including assessments and care plan development.
Provide education to patients and families on care plans, medications, and home health services.
Collaborate with physicians and clinical teams to ensure smooth care transitions.
Document all admission details accurately in the electronic medical record.
Ensure compliance with regulations and company standards.

What We’re Looking For

Current California RN License.
1 Year Home Health Experience.
Strong communication, organizational, and time management skills.
Tech-savvy and comfortable using electronic medical records.
Compassionate and committed to delivering exceptional patient care.

Flexible weekends - Contra Costa County
