Position Summary
The Maternal Infant Health Program (MIHP) Social Worker provides home-based and community-based psychosocial support to Medicaid-eligible pregnant and postpartum clients and their infants (up to 12 months old). The Social Worker partners with a multidisciplinary team—including a Registered Nurse, Nutritionist, and other professionals—to promote healthy pregnancies, positive birth outcomes, and safe infant development through education, assessment, and linkage to community resources.
Key Responsibilities
Client Services
- Conduct comprehensive psychosocial assessments for pregnant and postpartum clients using MIHP-approved tools.
- Develop and implement individualized care plans in collaboration with the client and the MIHP Nurse.
- Provide education, counseling, and crisis intervention related to family functioning, stress management, parenting, and maternal mental health.
- Perform home visits (and/or telehealth sessions) according to MIHP guidelines.
- Link families to community services such as housing, food assistance, transportation, childcare, and behavioral health.
Program Documentation
- Maintain timely and accurate client records in accordance with MDHHS MIHP documentation standards.
- Complete required risk assessments, progress notes, and outcome reports in electronic health record systems.
- Participate in case reviews, multidisciplinary meetings, and quality assurance activities.
Collaboration & Community Outreach
- Work closely with the MIHP team, healthcare providers, and partner agencies to coordinate services and avoid duplication.
- Advocate for families’ access to medical, behavioral, and social services.
- Participate in community health promotion events and outreach initiatives when appropriate.
Compliance & Professional Conduct
- Adhere to HIPAA and MDHHS confidentiality requirements.
- Maintain professional boundaries and demonstrate cultural competence in all client interactions.
- Follow agency policies related to mandatory reporting, safety, and emergency response.
Job Types: Part-time, Contract
Pay: $42,183.36 - $50,801.46 per year
