This includes all interactions with patients and their families, whether in person during office visits, via telephone, or electronically. Additionally, this position involves real-time charting to ensure an accurate and complete medical record for each patient visit.
Job Duties
- Accurately and thoroughly documents the encounter with the patient as it is being performed by the provider, which may include documentation in the following sections: problem list, history of present illness, review of systems, physical exam, assessment and plan, procedures and treatments performed by the physician, patient education, orders, medications, referrals, explanations of the risks and benefits, and instructions for self-care and follow-up.
- Identify the portions of the encounter that were scribed on behalf of the physician
- Communicates a summary of the patient-reported information to the provider prior to entry of the room or within the room with the patient present.
- Populates patient-reported information into physician-built EMR documentation tools for the specific reason for visits
- Prepares the progress note for the patient visit by manually bringing in physician-built EMR documentation tools for the following sections: history of present illness, review of the system, physical exam, assessment, and plan.
- Provides assistance with office procedures such as pap smears, colposcopy examinations, biopsies, etc. This includes obtaining specimens, labeling, and disposition to the laboratory, and explaining diagnostic procedures to patients as needed.
- Processes medication requests/changes/refills per office policy. This includes having knowledge of medications, including basic dosages, side effects, and interactions.
- Manages phone calls to patients regarding test/lab results and provides appropriate instruction for treatment, as directed by the physician.
- Schedule a follow-up appointment, provide referral information, or schedule patients for testing procedures and provide instructions. Provides patient instruction and performs phlebotomy as needed. Provides test results as directed by the Provider.
- Rooms and prepares the patient for examination. This includes reviewing histories, allergies, medications, the reason for the visit, and taking vitals.
Knowledge, Skills, and Abilities
- General knowledge and understanding of anatomy, the diagnosis process, pharmacology (drug classifications, side effects, interactions) physiology, lab values, sterilization techniques, and safe administration of medications.
- Ability to successfully perform venipuncture
- basic computer knowledge and navigation of an E.M.R.
Professional Experience
- High school graduate, GED, or equivalent required.
- One year of experience in a medical office
- Experience in a clerical role preferred.
- We will also provide the CMA training as a Clinical Scribe immediately upon hire.
