As a Women's Health Program Navigator RN, you’ll be the heartbeat of our women’s health program—connecting expectant moms and their newborns to the care and support they need from the third trimester through postpartum and beyond. Reporting to our Senior Director (or their designee), you’ll design, lead, and continuously enhance our Mom-Baby outreach and follow-up program.
You’ll be a friendly face in our clinics and on our hospital floors, helping moms navigate OB and pediatric care, eliminating barriers to care, and ensuring no one falls through the cracks. You’ll coordinate appointments, follow up with families after delivery, and partner with clinical teams to provide a warm handoff between services. Think of it as part care coordinator, part advocate, part educator—and all heart.
What You'll Do:
Guide and support patients from third trimester OB visits through delivery and postpartum follow-up
Manage the Mom-Baby follow-up program and reach out to new moms to help them engage in pediatric and postpartum care
Identify and address barriers to care—scheduling appointments, arranging services, or just providing a listening ear
Coordinate services across clinics and departments, ensuring patients move smoothly through our system
Visit and tour with patients not currently receiving OB or pediatric care, encouraging them to stay engaged
Collaborate with physicians, nurses, and clinic leadership to improve outcomes and ensure a seamless care experience
Partner with lactation consultants and childbirth educators to offer comprehensive support
Round on inpatients to assess satisfaction and gather feedback
Design and run postpartum follow-up call programs
Lead community outreach efforts related to pregnancy and newborn care
Help track and analyze data to improve outcomes and care quality
Support compliance, safety, and excellence at every turn
What You Bring (Minimum Requirements)
Bachelor of Science in Nursing (BSN)
Active Missouri RN license
BLS certification
Knowledge of women’s health and/or pediatrics standards of care and quality indicators
Outstanding communication and customer service skills—written, verbal, and presentation
Preferred, But Not Required:
Prior experience in labor & delivery, women’s health, or pediatrics
Certification as a childbirth educator
MSN or other graduate nursing degree
Bilingual in English & Spanish – a big plus!
