Queen Square Family Health Team (QSFHT) provides accessible, equitable, patient-centred primary care to its local community. A talented, interdisciplinary team works collaboratively with Queen Square Doctors, and together, we serve over 35,000 patients annually. We offer a wide range of programs and services including diabetes care, seniors’ wellness, palliative care support, mental health, maternal and children’s health and trans health. We are seeking a part time (15 - 25 hours per week) Registered Nurse to support patient care delivery and the implementation of programs that will optimize the health of our patients. Reporting to the Manager of Nursing Services and in collaboration with the Nursing Team, this position will work as part of the team providing direct primary care for patients.

Responsibilities
As a registered and licensed registered nurse, you will:
· Participate in the care of the patients in the maternal and children’s health, breastfeeding, maternal mental health, gender affirming care and palliative care programs as well as other programs as needed
· Complete holistic health assessments, including a health history and physical examination and contribute to developing care plans
· Monitor the ongoing therapy of patients with chronic illness by providing effective pharmacological, complementary, or counseling interventions
· Work collaboratively with other team members to manage the needs of patients
· Provide home and virtual visits when needed
· As appropriate, share results of assessments and interventions with physicians, team members, hospital, home care, and others in the circle of care
· Liaise and collaborate with families and community stakeholders
· Develop and evaluate educational resources, presentations, workshops, for the internal team and for patients and their families
· Collaborate with nursing team members including Registered Practical Nurses and Nurse Practitioners to meet the needs of all patients
· Recommend and arrange for specialist referrals and appointments in consultation with physicians and team members
· Exercise reasonable care and caution in protecting confidential and sensitive information related to clients and personnel
· Perform other related duties as assigned by the Executive Director and/or Manager of Nursing Services
As a member of the Queen Square Family Health Team, you will:
· Work with community and hospital partners to strengthen Queen Square’s program and advance patient care services
· Develop, implement, and improve patient programs
· Contribute to the overall success of the Family Health Team
· Identify and be involved in quality improvement activities, including the collection of data for measurement, reporting and evaluation
· Participate in team building and team meetings
· Support and contribute to a healthy workplace that embraces lifelong learning, diversity, clinical teamwork, privacy, and adherence to professional standards of practice
Qualifications
· Current registration with the College of Nurses of Ontario (CNO) as an RN
· Current membership with the Registered Nurses’ Association of Ontario (RNAO) and evidence of liability insurance is required
· Baccalaureate Degree in Nursing
· CNA certification in Perinatal Nursing or Hospice Palliative Care Nursing or Gerontology Nursing an asset.
· Exposure/certification in fundaments of Gentle Persuasive Approaches, U-FIRST!, P.I.E.C.E.S., Montessory methods training, certificates in gerontology and mental health, person-centred care service provision an asset
· Experience in providing palliative care an asset
· Recent related nursing experience; Prefer working experience in a community-based or hospital-based clinical team delivering related programs
· BCLS/CPR/First aid trained
· Experience working as part of an interdisciplinary team
· Comfortable with electronic documentation and use of technology in the workplace
