Position Summary
Works closely with the assigned care team, including the nursing team from The Center for Maternal Health Program, to identify patients who would benefit from services. Provides essential support in assessing risk factors, coordinating services, and facilitating access to resources that enhance maternal health care and preventative support. Assists in ensuring communication and collaboration among patients, families, and healthcare providers to promote continuity of care and positive health outcomes.
Responsibilities:
Essential Functions
- Assists in identifying eligible patients for The Center of Maternal Health, guiding them through the enrollment process, and supporting successful program completion and graduation based on established criteria.
- Coordinates patient support.
- Collaborates with teams to track progress towards interventions to optimize patient outcomes.
- Supports maternal patients by connecting them with essential resources.
- Educates patients about the healthcare system, including insurance coverage, prenatal and postpartum care options, and available resources.
- Evaluates the needs of patients and families to determine gaps in existing resources and support systems.
- Assists in creating individualized strategies that address patient needs.
- Builds effective working relationships with support personnel to engage patients and families in collaborative problemsolving advocacy.
- Collaborates with teams to track patient progress towards goals, ensuring that patients receive timely interventions and follow-ups.
- Participates in maternal health program evaluations, outreach events and improvement initiatives to promote patientcentered care and overall well-being.
- Adheres to policies and procedures to ensure patient safety and program compliance including punctual and consistent
attendance, following organizational guidelines and regulatory standards.
- Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA, and other federal, state, and local standards.
- Maintains compliance with all Orlando Health policies and procedures.
- Performs all other duties as assigned.

Qualifications:
Education/Training
Education:
- Bachelor’s degree in public health, healthcare administration, human services, or a related field required. Preferred Training & Certifications:
- Training in maternal health, reproductive health, or community health services.
Licensure/Certification
None
Experience
Two (2) years of direct clinical experience with an emphasis on population health.
Bilingual Preferred
