Job Summary
Provides integrated support services and information to pregnant and new families with a focus on healthy pregnancy, healthy babies and the transition into parenthood, assisting them to navigate the health care system, other systems, parenting and connecting with other pregnant people and new parents. Under the direct supervision of the Pregnancy Outreach Program Coordinator the Social Worker will work as a part of a team.
Qualifications:
- A Bachelor of Social Work is preferred or a combination of a social work or human service diploma and one year’s work experience.
- Two years of recent related experience including supervisory or volunteer management experience
- An equivalent combination of relevant education, training and experience will be considered.

- Demonstrated ability to work effectively with multi-disciplinary case management teams.

- A high level of tact, diplomacy and professionalism.
- A high level of discretion and adherence to privacy legislation and policy.
- Proficient in basic computer applications, including Microsoft Office.
- Excellent time management skills.
Key Responsibilities and Duties:
- Provide individual support, resources and information to pregnant people and new families
- Apply trauma-informed practice to provide culturally sensitive consultation, and referrals for identified health and lifestyle risks related to pregnancy, childbirth, and the postnatal period
- Plan information sessions for drop-in programs in conjunction with the
- nutritionist coordinator
- Assist and facilitate a weekly drop-in group and the Postpartum Depression & Anxiety Peer Support Group
- Provide services such as in-home visits and transport clients to and from appointments, as necessary.
- Assist families in connecting with NOFCS programs such as Mental Health, Family Support & Father’s Support, Early Childhood Development and other community programs.
- Network and liaise with community agencies and resources.
- Cultural knowledge and understanding of Indigenous families
- Ability to work independently and as a team. Be reliable and self-motivated.
- Must have good organization, communication and conflict resolution skills
- Ability to work in fast paced environment often dealing with family crisis situations.
- Flexible work hours (evening groups as needed ex. Prenatal classes).
- Assist in evaluation of programs and services offered through the Pregnancy Outreach Program
- Other duties, as required.
Program Administration:
- Maintains client records and appropriate documentation according to Records Management Guidelines and agency policy. Ensures client records are securely stored. Ensures all required documentation is complete and accurate.
- Maintains and submits accurate statistics and reports as required by the funder, the Society or as requested by the Program Manager.
- Develops and maintains a collection of resource material for clients.
- Ensures that policies and procedures are adhered to in the delivery of services.
- Submits employment records such as Timesheets, Mileage Reports and Professional Development forms to the Program Manager in a timely fashion.
Community Role:
- Initiates referrals and liaison with community agencies as required by each client’s case.
- Represents the Society positively and professionally in the community and in all activities.
Other
- Performs other duties as directed.
Additional Job Information:
- A criminal record check for working with vulnerable people is required.
- A valid Class 5 Driver’s license and availability of personal vehicle for work is required.
