Position Summary
The
Reproductive Health Nurse will provide clinical oversight and curriculum-based education to the pregnant and parenting population and other youth of age in the MATCH Program of youth in Baltimore City Department of Social Services (BCDSS) Foster Care.
Primary Responsibilities
- Coordinate holistic healthcare for children in MATCH program by partnering with the foster parents, child welfare caseworkers, biological parents, managed care organizations, and healthcare providers.
- Ensure the health care needs of youth in foster care who are pregnant and/or parenting are being met in a timely manner with a minimum of monthly follow up during the prenatal period.
- Identify youth at risk for pregnancy-related complications, parenting difficulties, and monitor closely.
- Provide curriculum-based reproductive health education, resources and recommendations to all identified youth in out-of-home placement with Baltimore City Department of Social Services to maintain wellness.
- Attend internal and external HCAM and BCDSS meetings as identified.
- Complete home visits as necessary to assess children’s health needs and provide education to caregivers.
- Assist foster care families with navigating the Health Choice system.
- Prioritize care and contacts based on urgency, including ability to multi-task with accuracy.
- Document case management assessments, activity, treatment plans and resolutions in the MATCH database as well as BCDSS database.
- Participate in staff, community, professional and inter-agency meetings and conferences.
- Other job duties as assigned.
Minimum Qualifications
- A Bachelor’s degree in nursing from an accredited college/university.
- Current licensure as a Registered Nurse in the State of Maryland.
- Relevant clinical experience in Maternal/Child Nursing, preferably in a public health/community setting.
- Experience in public speaking.
- Good planning and organizational skills.
- Well-developed interpersonal and communication skills.
- Professional appearance and demeanor.
- Computer literate, specifically MS Word, Excel, Outlook, and Gmail.
Compensation
In compliance with Maryland's wage transparency requirements, the compensation for this salaried position includes a base pay of $74,000 annually.
