Cayaba Care's mission is to improve pregnancy, birth and the postpartum journey for women and their families by reimagining how maternity care is delivered. Our team is focused on building trust-based engagement and wrap-around support for expectant moms at home, in person and virtually, to ensure they have all the social support they need for a successful and joyful pregnancy journey. We leverage technology to support our virtual engagement that allows us to meet all our moms where they are.
We ensure that our members do not feel alone during their pregnancy journey, and that they have a trusted resource for support, resources, and information throughout their pregnancy, delivery and well into postpartum. Cayaba Care does not replace the primary obstetrician or doctor’s relationships. Instead, we support expectant mothers with all other support services that are traditionally not available through their doctors’ offices.

The Role
We are seeking a part-time Registered Nurse (RN) to help bridge the gap between clinical care and community support for new mothers and NICU families. In this role, you will be a trusted clinical partner to the families we serve and our Maternity Navigators (MNs) that guide our members through their postpartum journey. You will collaborate with a multidisciplinary team to assess NICU families' clinical needs post-discharge, connect families with local resources, and address any postpartum concerns.
As part of our DC expansion, this role will help shape how we serve the local community, ensuring our care model meets the unique needs of families in Washington DC. This role will report to the Director of Care Operations.
Typical Hours: 3 days per week 9-5P with an opportunity to transition to full-time
Responsibilities:
Clinical Monitoring & Escalations
- Conduct in-home assessments for moms and infants within 48 hours of the infant’s discharge from the NICU
- Conduct telemedicine visits at set points in postpartum period to identify unmanaged clinical needs and educate members on more complex clinical topics as needed
- Assess members that report Urgent Maternal Warning Signs or high blood pressure values, and help them navigate to the most appropriate care setting
- Review blood pressure and EPDS values and follow-up on irregular values that have not already been escalated
- Serve as a clinical liaison between Cayaba Care and the member’s obstetrics provider or primary care physician, documenting visit summaries and escalating all unmanaged clinical needs reported to the Cayaba Care team
Care Quality & Training Support
- Be a voice for the community, bringing insights from your experience working with mothers and families in DC to help shape Cayaba Care programs that truly meet their needs
- Help train Maternity Navigators (MN) on prenatal and postpartum topics to ensure every Cayaba Care member receives the same high-quality, evidence-based education
- Foster a collaborative environment between MNs and clinicians, encouraging proactive discussion of cases during care team huddles and sharing of knowledge on health education and community resources as needed
Qualifications:
- Bachelor of Science in Nursing
- Active compact state or Washington DC nursing license
- Located in the DC area, ideally in or highly familiar with Marshall Heights, Twining, Ivy City, Petworth, or surrounding areas
- Minimum of 5 years of experience providing postpartum care in an outpatient pediatric, OB/GYN, maternal-fetal medicine, or similar ambulatory care setting.
- A great listener and strong communicator who is highly skilled in providing compassionate, culturally informed care and empathetic inquiry techniques
- Excellent ability to turn problems into solutions and stay organized in a flexible and fast-paced environment
- Comfort building working relationships and patient relationships virtually, and excitement to collaborate with a team of Maternity Navigators
- Tech-savvy and able to quickly learn to document interactions and communicate effectively in our technology platforms such as AthenaOne, Welkin, and MacOs
