CARE NAVIGATOR Adagio Health Inc is looking to hire one (1) full-time Care Navigator for its Erie office
- We are looking for compassionate and driven individuals to join our team as a Care Navigator. This role is integral to our mission of providing comprehensive support and care to a diverse range of clients, including those with specific needs such as HIV, veterans, and others requiring specialized services. RESPONSIBILITIES Client Assessment and Support: Conduct thorough assessments of client needs, focusing on behavioral health, food insecurity, transportation, housing, and other essential community services. Knowledge Development: Gain in-depth knowledge of available community support services and programs, including the referral process for each. Veteran-Specific Expertise: Become adept in military benefits, documentation, and health insurance navigation to support our veteran community effectively. Referral Management: Provide well-coordinated referrals, both internally and externally, ensuring a seamless transition for clients. Barrier Identification and Resolution: Assist clients in identifying and overcoming barriers to service access, utilizing local resources and supports to optimize care. Educational Support: Offer education and resources to clients as needed, enhancing their understanding and engagement in their own care. Documentation and Data Management: Meticulously document all client interactions and data, participating in data analysis and reporting. Quality Improvement Participation: Engage in initiatives aimed at enhancing the quality of services provided. Collaboration and Outreach: Work closely with clinical and program staff, maintaining open communication lines. Conduct outreach to build and maintain relationships with community partners. Veteran Mental Health First Aid (MHFA): Obtain and provide Veteran MHFA to partners and veteran audiences. Follow-Up: Ensure continuous care by providing consistent follow-ups with patients, clients, and partners. Teamwork: Collaborate with various community service providers, including clinical, nutrition, and ancillary support services. Technology Utilization: Employ virtual/tele-health and texting program equipment for remote client interactions. Additional Responsibilities: Be adaptable to perform other duties as assigned. QUALIFICATIONS FOR A CARE NAVIGATOR Accredited degree in Social Work, Public Health or related field. Experience in case management/care planning, customer service, and enrollment process for medical insurance products, preferred. Must be able to comfortably work with Microsoft office products.
