The purpose of this position is to provide advocacy and support to our pregnant and parenting women by working with the Health Bureau and partnering with local, state and national partners on specific public health issues. This position will act as a liaison between clinical or community health staff and service users; identifying and/or assisting in coordination and/or promotion of Bureau events or participating in community events; promoting Bureau services; collecting data and responding to public health emergency situations. The ability to translate written and verbal communications between English and Haitian Creole, is essential to effective communication with service users (i.e. residents, patients, community members, etc.) who speak Haitian Creole as their first/primary language.
This is accomplished through home visitation, supportive follow-up, and administrative services, assisting the Healthy Moms Healthy Babies team, to provide services for improving the health and wellness of women and infants in the City of York. This position will provide communication support in group and individual settings, including but not limited to community events, meetings and other Bureau activities. Networking with community providers to understand services available to service users; connecting service users to resources in the community; advocating for health concerns of service users; assisting in the creation and dissemination of culturally and linguistically appropriate materials to targeted populations or city residents in general. An understanding of cultural barriers and general concerns is an expectation.
Providing outreach to the community on specific public health topic areas. This is accomplished by conducting outreach to community stakeholders and service users to facilitate participation in services/programs. Disseminating accurate, culturally and linguistically appropriate information to diverse populations and service users. Identifying issues of concern or needs of patients/residents. Advocating for patient/resident health concerns, serving as a liaison between Bureau staff and patients/residents. Connecting service users to resources in the community.
Collaborate with community partners. This is accomplished by identifying community partners and resources/services available to service users. Attending meetings with local, state and regional partners. Maintaining and establishing working relationships with resources and professional partners. Representing the Health Bureau by engaging in community events to promote public health programs and services.
Raise awareness among residents of public health issues and interventions. This is accomplished by assisting in the creation of culturally and linguistically appropriate materials for the public on a variety of public health issues. Disseminating information via community presentations, flyers, social media, etc. to reach targeted audiences or service users. Communicating public health information to various audiences, including community organizations and service users. advocates for patients/residents for appropriate services.
Participates in workforce development opportunities to develop skills and up-to-date practices in specific program areas. This is accomplished by attending relevant conferences, webinars, and trainings to advance skills and knowledge. Participating in staff training around specific programs areas, i.e. emergency preparedness. Engaging in planning meetings and staff activities to assess and set the course for goals and objectives.

High School Diploma or equivalent required.
Experience under and including one (1) year.
Valid Class C Driver’s license issued by the Commonwealth of Pennsylvania
Bilingual (English and Haitian Creole) - speaking, reading and writing
All persons who shall become employed by the City shall be or become a resident of York County, or any County contiguous to York County (Adams, Cumberland, Dauphin, Lancaster, Hartford, Baltimore, and Carrol Counties), within six (6) months from the date the employee achieves permanent employment status.
