Responsibilities
- Transports Sisters to and from doctor's offices, laboratories, and hospitals for their medical appointments.
- Provides supervision to Sisters and/or assists with personal hygiene and activities of daily living while promoting independence as necessary to meet individual resident needs.
- Administers medications and treatments, monitoring for changes and maintaining current and accurate medication records.
- Assist Sisters with ambulation and transfers to activity events.
- Responsible for testing and setting up isolation precautions, meal deliveries, etc.
- Greets visitors, answers questions, and refers to staff members or departments to ensure that visitors are promptly served.
- Assist Sisters with transportation to activities and appointments within the community to ensure safety and timely arrival.
- Responsible for accurate reporting and documentation to promote effective communication and thereby provide optimal continuity of care.
- Maintains compliance with the Sisters of the Humility of Mary (SHM) policies and procedures in accordance with governing laws, regulations, regulatory agencies, accreditation bodies, and community standards.
- Consistently demonstrates individual and team behavior that supports person-centered and SHM philosophies of care.
- Participates in the residents' care plan to enhance the quality of life, resident rights, privacy, and confidentiality.
- Communicate with team members to promote effective communication and coordination of services.
- Advises and coaches as opportunities arise. Reports noted performance or safety issues to the Director of Sister Care.
- Conducts planned and spontaneous activities with individual Sisters, small groups including dementia programming to promote the highest level of resident functioning, while promoting self-esteem.
- Promotes a safe, calm, and healing environment.
- Has knowledge of and implements emergency measures to ensure Sister safety and comfort in the event of an accident, fire, or disaster.
- Participates in required training and annual in-services meeting all assigned deadlines. Applies education to practice.
- Seeks assistance from the Director of Sister Care for care planning purposes or any other needs as they are identified.
- Completes incident reports for falls or any event that is not a part of normal daily operations.
- Floats throughout the healthcare unit, Assumption, and Humility halls to meet the needs of Sisters and to promote teamwork.
- Provides care and utilizes supplies in a fiscally responsible manner.
- Participates in the "on call" rotation.
- Maintains attendance as defined by HR policy.
- Consistently upholds the mission, values, and behavioral standards of the SHM that is inclusive of the HEART core values.
- Maintains confidentiality and is respectful of the diverse needs of others.
- Demonstrates flexibility by accepting other duties as assigned.
Education and/or Experience: Licensed Practical Nurse experienced in personal care, assisted living, and geriatric nursing preferred.
Computer Skills
To perform this job successfully, an individual should have knowledge of internet software.

Valid current Pennsylvania practical nursing license in good standing
CPR Certification approved by the American Heart Association

Valid driver's license
