Summary
- Seeking BE/BC general urologist
- Practice includes 16 urologists and 6 advanced practice providers currently
- Clinic opening will support primary office in Arlington Heights, with Hoffman Estates satellite
- Possibility of growth into Schaumburg
- Call will be taken at Northwest Community Hospital
- Call of 1:4

Comensation And Benefits
- Comprehensive medical, dental, and vision benefits that include healthcare navigation assistance and medical coverage at 100% (after deductible) when utilizing a Duly provider. Employer provided life and disability insurance

- Access to a non-qualified deferred compensation plan once eligibility requirements are met
- Eligible for sign-on and relocation bonus and loan forgiveness option
- Catalyst stipend of $2,500/month while completing training
- The anticipated base compensation range for this position is $425,000 - $525,000, depending upon the applicant’s qualifications and experience. In addition to the base compensation, this position may also be eligible for a productivity bonus, sign-on bonus, loan repayment assistance, catalyst training stipend, and relocation bonus equaling a total target cash compensation that may be between $450,000 - $675,000.
