Position Overview
We are seeking a
full-time Assertive Community Treatment (ACT) Nurse (LPN) to join our team. In this role, you will provide clinical services to clients receiving ACT team support, focusing on medical and clinical care while fostering the highest quality of life for each individual.
Location: Raleigh, NC
Key Responsibilities
Clinical Services
- Monitor all medical and clinical aspects of services delivered to clients.
- Provide medication monitoring, psychopharmacologic care, and medical services within the scope of practice of your license.
- Deliver crisis assessment, symptom management, and intervention services.
- Support individual counseling, psychotherapy, and substance abuse treatment.
Service Coordination
- Work collaboratively with the ACT team to assess each client’s clinical status and response to treatment.
- Coordinate care with primary healthcare providers and other medical staff, including state and community inpatient facilities.
- Assist in planning, scheduling, and implementing client services, including developing and evaluating transition plans for clients moving in and out of services.
Administrative Responsibilities
- Document treatment and goal plans, activity notes, and service records as required for medical records.
- Identify and recommend evidence-based practices to enhance care delivery.
- Participate in on-call crisis response duties on a rotating basis.
Qualifications
- Licensure: Must be a licensed practical nurse (LPN) authorized to practice in North Carolina by the NC Board of Nursing.
- Skills and Training:
- Proficient in implementing evidence-based or emerging best practices in care delivery.
- Strong collaboration and communication skills to coordinate with medical staff and community resources.
- Ability to provide compassionate and effective care in diverse community settings.
- Additional Requirements:
- Must be free from all illicit substance use.
- Ability to lift no more than 20 pounds (Light Work).
- Must have reliable transportation and a valid driver’s license.
Compensation
- Rate: starting at $25.02
