About The Role
Sydney Local Health District’s Community Health Centres have a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing care services designed to improve and support the health and wellbeing of individuals and communities.
The Wellbeing and Health In-reach Nurse (WHIN) Coordinator will provide safe, compassionate, quality, person centred care and work in collaboration with children, young people, families, carers and the identified educational facilities along with a multidisciplinary team to achieve identified goals and outcomes. The WHIN coordinator is responsible for facilitating identification of health, wellbeing and social needs and the coordination of appropriate early intervention, assessments, and referral to services for school-aged children and young people attending designated primary and secondary schools.

Ideal Candidate
- Current NSW Driver's Licence (P1 and P2 acceptable).
- Current authority to practice as a Registered Nurse with AHPRA.
- Post-Registration qualifications and at least 3 years experience working in the clinical area of specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.
