Job Description

Capitol View Transitional Care Center is a state-of-the-art true transitional care unit (TCU) located in Regions Hospital (we are part of the HealthPartners organization as a separate nonprofit organization.) We provide quality care to patients from 18+ years of age who are in need of a short-term stay. Our average length of stay is 17 days. As a Minimum Data Set Nurse, you will be responsible for the following job duties and responsibilities:

Develop, implement and evaluate resident assessment care plans with collaboration of other health professionals to optimize patient well-being.
Assessing and monitoring patient health to meet CMS, MDS/RAI, state and federal compliance requirements.
Oversee the completion of an MDS that is comprehensive, accurate, standardized and reproducible.
Reviews and distributes Quality Indicator data and other reports to improve MDS accuracy and compliance.
Provide educational training related to Medicare, MDS/RAI process to staff as needed.
Performs other duties as assigned.

FTE: 0.9; 36 hours per week

Work Schedule: Monday-Friday; Day shifts

Salary: $33.61 - $50.42 hourly

Required Qualifications

Current Minnesota RN or LPN License
Maintains current CPR certification
Must complete Minimum Data Set class within 1 year of employment

Preferred Qualifications

5 years of MDS/RAI work experience

Benefits

Benefits are available for non-union staff working 24 hours (0.6 FTE) or union staff working 30 hours (0.75 FTE) per week and are effective the first of the month after 30 days of employment.
