PRIMARY DUTIES:
Centering Session Coordination
Maintains FHC Centering counts data collection
Maintains database for session volume and patient outcomes
Schedule patients into Centering groups by month of Expected Date of Confinement (EDC)
Performs call backs on all missed Centering prenatal appointments and schedule GAP visits in traditional clinic
Assists in facilitating English Centering sessions
Recruits patients for Centering groups including accepting warm handoffs
Collaborates on grants for Centering funding
Coordinating Centering snacks
Ensures transition to traditional prenatal care for the rare Centering patient who elects not to continue in Centering
Maintains Centering session stocks and supplies
Keeps Centering OB flowsheets up to date with labs/ultrasounds/referrals
Keeps Centering patient purple cards up to date
Coordinates FHC Centering efforts with CHI national and regional coordinators
Alongside management, pairs co-facilitators with providers for group assignments
Assists in requesting schedule templates are built in EPIC and relay changes when necessary
Responsible for scheduling Centering Pregnancy guest speakers (dental, WIC, IHM)
OTHER DUTIES:
Nursing ,
Monitors timing for, schedules and administers all 1 Hr GCTs, Tdap, 3Hr GTTs, Quad Screens, Flu Vaccinations, Rhogam administration for Centering patients
Initiates referral paperwork for Alere progesterone program for Centering patients and some non-Centering patients with risk factors
Notifies patients of prenatal lab results
Schedules prenatal ultrasounds for Centering patients
Training
